IPCA Care Management Program - Webinar Series

This webinar series will focus on reaching different audiences for building support for care management programs and developing an infrastructure that can be used in the current fee-for-service (FFS) environment, with strategic thinking on how building a solid care management strategy will support value-based care. A specific focus will be on developing processes to care for patients with diabetes and hypertension.

Health Management Associates (HMA) will provide a four-webinar series and additional technical assistance (TA) spanning over the first half of 2021. Each of the webinars will be recorded and available after the live training. Idaho health centers who would like additional support will have the option of TA with HMA.

 

Webinar 1: Care Management Best Practices, Return on Investment and Options for Financial Sustainability in the Transition to Value Based Care

February 22, 2021 - 12-1pm MST

  • Overview of best practice care management models (design and staffing), effectiveness, and ROI
  • Review options for financial sustainability and transitioning to VBC

Webinar 2: Population Health and Care Management – Using Data to Focus on Populations of Greatest Need

Webinar 4: Maximizing Fee-for-Service Care Management Revenue and Preparing for Value-Based Reimbursement

May 24, 2021 - 12-1pm MST

  • Current FFS billing around care management; CCM, TCM, and what are you already getting paid for
  • A look to the future - transitioning to Value Based Care and Alternate Payment Methods

Who should attend? CFOs, COOs, Care Managers/Coordinators, Revenue Cycle/Billing Teams.

To Idaho community health centers, this series is free. Other State PCAs and CHCs may also attend the full series at $50 per person.

Payment & Cancellation Policy

Registration fee payments must be received prior to the training. Payment may be made via credit card or check and mailed to IPCA at 1087 W. River St., #160, Boise ID 83702.  Payment may be grouped for multiple participants. 

Cancellations must be made in writing. Cancellations made 30 days prior to the event will be given a full refund, cancellations made 14 days prior to the event will receive a 50% refund, cancellations made less than 13 days prior to the event will not be refunded.
